Integrated Nurse Program
Through the Integrated Nurse Program, specialized nurses in their field coordinate your care, help you make informed decisions about your course of treatment and locate best-in-class providers, including nationwide Centers of Excellence that meet high standards for providing high-quality and cost-efficient care.
Typically, a specialized clinical nurse will serve as your single point-of-contact and provide personalized support throughout your treatment and recovery. Best of all, your dedicated nurse can enlist other specialists to help you coordinate care across every spectrum of your life. For instance, if you will be hospitalized and have small children, a social worker can be engaged to ensure that your child care needs are met.
Additional services and support, including behavioral health specialists, can also be initiated so that you can focus on your recovery. Specialized conditions in which you may wish to enlist personalized service and support include:
- Case management when hospitalized
- Diabetes
- Heart disease
- High blood pressure
- Lower-back issues
- Pregnancy
- Neonatal issues (UnitedHealthcare only)
- Kidney disease
- Transplant services
If you are newly diagnosed with a serious health condition, a specialized program nurse will contact you and offer to coordinate your care. Your information will never be shared with anyone at the Firm and your confidentiality is assured. You may decline participation in the Integrated Nurse Program at any time.
